Insurance & Billing

We have contracted with most health plans licensed in the state of Connecticut. We will bill your insurance as a courtesy and will ask for your current insurance card, as well as photo identification, at the time of your service.

Below is a list of the most common insurance plans which we accept. If yours is not listed, please contact your insurance carrier or our billing service, Change Healthcare at 844-820-3999 for confirmation. While this list is updated regularly, it is your responsibility to check with your insurance provider for coverage and benefits. Each time you visit our office, it is necessary for us to copy your current insurance information as well as a copy of your photo identification. If your exam is covered by insurance, as a courtesy, we will file your insurance claim for you. Your insurance may require you to pay a co-pay, and for your convenience, we accept most major credit and debit cards, personal checks, and cash. All unpaid balances after insurance will be billed to you monthly. Patients are responsible for payment of their medical care within a reasonable period of time.

If you do not have health insurance and/or cannot pay for the service in full, we will be happy to assist you. On January 1, 2017, we will be changing our billing company to improve our service to you. If you have questions about your bill, and your exam was before January 1st, 2017, please call Origin, at 1-888-747- 3016. If your exam was after January 1st then call Change Healthcare at 844-820-3999.

Insurance pre-authorization

Many insurance plans require pre-authorization before a radiological procedure may be ordered, and the referring physician will normally obtain that for you. Usually the physician must provide the insurance company with clinical information concerning your medical condition and prior treatment, if any. Guilford Radiology is happy to assist physicians in obtaining pre-authorization if needed. Please work with your referring physician and insurance company to ensure your preauthorization number, when required, is received by our office before your scheduled radiological exam.

Without your insurance carrier's preauthorization number you may become financially responsible for any services rendered or your appointment time may need to be rescheduled.
